The Health Equity Action Team (HEAT) is a dedicated initiative working to advance health equity in our community. By addressing social determinants of health - the conditions where people live, work, and age - HEAT aims to eliminate disparities in   healthcare outcomes. Our work recognizes that health goes far beyond medical care alone; factors like housing, education, income, and neighborhood conditions profoundly impact wellbeing.

How Can you Join:

    • If you are interested in attending our events or simply hearing about the work we do, please email monserrat.m.santos@kp.org and you will be added to our listser
